Date of Accident
Name, Address, and Phone Number of Claims Representative
Claim Number

Workers Comp

When you come in for your appointment, the following information will need to be readily available:

Name, Address, and Phone Number of Employer
Name and Address of WC Insurance Company
Name and Phone Number of Claims Representative
Date of Injury
Claim Number
